Over the past 30 years, we have designed all manner of personnel forms for HR administration. These forms have all been used - and in many instances have been the key documents - in hearings and court cases. All forms conform to EEOC, ADA, and FEHA regulations, attempt to reestablish the at-will doctrine, and are constructed to establish the employer-employee (or applicant) relationship.
The PERSONNEL REQUISITION form is designed to assist in proper staffing and requires justification for any new position or addition to staff. Along with job descriptions, it helps define staff which is absolutely needed, not just what a manager wants.
The ORIENTATION CHECKLIST, emphasizing the at-will doctrine, is designed for supervisors and managers to be certain that the new employee understands all pertinent policies and procedures, grade, compensation, and position. This form can be modified to meet your company's needs.
The NONEXEMPT PERFORMANCE APPRAISAL, with mandatory comments sections, are weighted and scored evaluation forms designed and refined over the past 15 years. Used properly (with care) they have proven their value in evaluation, objectives-setting, and compensation administration. The Exempt review form is no longer available. This form is similar to the Combined Form with some truly exempt criteria removed. Page 2 is not represented, having to do with a comments section and objectives-setting.
In 1993 and revised in 2002, Winning Associates introduced a COMBINED EXEMPT AND NONEXEMPT REVIEW FORM, six pages long including objectives-setting worksheets.
The EMPLOYEE CONSULTATION ("WARNING") REPORT, is an extremely important document to be used in disciplinary situations, including those encompassing suspension and termination. Without a doubt this has been one of our most "popular" forms because of its simplicity, fairness to the employee, and reaffirmation of management's discretionary powers.
The OCCURRENCE MEMO can be used for both positive and negative situations. On the negative side, it would be a precursor to the Consultation Report.
The EXIT INTERVIEW REPORT is to be used for all employees who are leaving the organization. It is a questionnaire to be signed by the employee, and is designed to determine weaknesses in the organization as well as to protect the company from future claims. (Note: In using this form, the organization should be willing to commit some action regarding negative information received, although a disclaimer has been added allowing the company to take or not take action at its sole discretion.)
